Companion Package

INCLUDED IN YOUR FEES:

Your flights to and from San Diego
Transportation to and from the San Diego airport to the hospital
A Patient Facilitator to assist and support you during your stay at the hospital and hotel
Pre-operative tests: EKG, X-Ray, blood test and evaluation by an internal medicine specialist
Hospital fees, surgeons fees, anesthesiologist fees
2 days recovery in a private hospital room with a view
2 days recovery at the 5 star Hotel Grand Tijuana
Access to free long distance calling while at the hospital
Antibiotics and pain medication during your recovery
A nutritionist program to help you achieve your ideal weight loss
24 hour phone service to answer any questions or to address any problems
Ongoing monthly support group meetings and clothing exchange
2 week free membership at Fitness Focus
Referral to a clinical psychologist familiar with obesity issues, if required
Pre and post-operative support and care
Professional glamour photo session once patient has reached their goal weight**

DOES NOT INCLUDE:

Additional hospital expenses if patient has to stay in the hospital for a longer duration
Expenses due to complications
Doctor’s fees for appointments before and after the surgery
Additional nights at the hotel
Food and drinks during recovery time at the hotel for patient and travel companion
Companion stays with patient in provide room in hospital and in the hotel during the recovery period (Extra fee will be charged if you bring a companion with you.)
Weight Loss Forever owns and has exclusive rights to all photographs**

Comfort Package

Weight Loss Forever now offers a Comfort Package that covers any costs associated with any complications that may arise resulting from surgery. This package covers doctor/specialist fees for appointments before and after the surgery, extra prescriptions, special studies or tests related to the procedure and flight changes – if required.

DOES NOT INCLUDE:

Additional hospital expenses for non-surgery related health problems
Coverage for any complications that may arise while in the United States or Canada
Coverage for expenses due to surgery cancellation for any reason

Additional Patients' Services Package

An additional service package applies to cover the cost of extra staff, specialists and additional tests for the following:

Patients who are designated as high risk (A high BMI over 50 or any other risk health conditions)
Revision surgery from a prior bariatric procedure
Additional concurrent surgeries (i.e.: cholecystecomy (gallbladder removal), hernia repair)

DOES NOT INCLUDE:

Additional hospital expenses for non-surgery related health problems
Coverage for any complications that may arise while in the United States or Canada
Coverage for expenses due to surgery cancellation for any reason.

Please Note: Weight Loss Forever purchases insurance with your airline tick, which covers: single trip, multi-trip emergency, medical, trip and baggage. If you fail to submit the appropriate form(s), you will be billed for the insurance claim amount.

@ Larissa, you can actually go onto our website and fill out the 'Am I a Candidate' portion to get the ball rolling. Once you do this, you will be assigned a facilitator who will be in contact with you. Can't wait to speak to you!!
